"Aus gegebenem Anlass" soll sagen, dass ich kürzlich eine Patientin zu behandeln hatte, die bei verschiedenen Institutionen Rat gesucht  hatte, ehe man ihr riet, mal bei mir nach Muskelübungen zu fragen. Sie hatte eine Atemmaske und eine Schiene, aber keine Behandlungsanweisungen für neuromuskuläre Funktions-störungen respektive -übungen bekommen und sah keine Chance auf Besserung ihres Zustandes. Ich habe ihr aus meinem im Buch erschienenen Übungssammlung die allgemeinbekannten entsprechenden Übungen zusammengestellt und sie unterwiesen. Nach ein paar Wochen eröffnete ihr ihr Mann, dass der Zustand sich deutlich besser anhöre. In meinem Buch sind die Übungen mit Beschreibungen für den Therapeuten und Anleitungen für den Patienten abgedruckt. Ich geben hier nur die Patientenanleitung wieder.

"On the opportunity of a current situation " will say that I shortly had to treat a patient, who had asked for advice at different institutions before someone had suggested to ask me for muscle exercises. She had got a breathing mask and a splint but no treatment instructions for neuromuscular function disorders and did not see a chance for improvement of her situation. From my book with the Exercise collection I have compiled and demonstrated for her the commonly known respective Exercises. After some weeks her husband  revealed to her that the situationh sounded much better. In my book the exercises are quoted with descriptions for the therapist and instructions  for the patient. Here I only cite the patient instructions.

A U F Z U G (Anleitung)047

Die Muskeln hinten im Mund sollen die Beweglichkeit und Kraft bekommen, die sie brauchen, um das ‘K’ zu sprechen, das Essen beim Schlucken in die Speiseröhre zu drücken und die Atmung richtig zu lenken.      Dazu wird eine Flocke, ein Cornflake, auf die Stelle gelegt, die wir uns gemerkt haben, in der Mitte ganz hinten am Gaumen, wo der harte Rand ist.          Das ist die Last, die unser Lastenaufzug nun transportieren soll. Aufwärts und wieder abwärts und wieder hoch und runter. Jedes mal, wenn das Flake oben ankommt, stößt es an genau die Stelle, die wir eingeübt haben.       Wenn es sich aufgelöst hat, kommt das nächste dran.   Mindestens drei Flakes müssen so verbraucht werden, oder die Übung wird so oft gemacht wie verabredet.

ELEVATOR (Pat.-Instruction)

The muscles in the back of the mouth shall get the power they need to speak the ‘K’sound, to avoid choking the meal into the gullet, and to guide the breath correctly.        To work this out, you put a flake (cornflake) on the spot we have tried out, in the middle quite at the end of the palate where the hard rim is.      This is the load which our goods lift will transport. Upwards and downwards again, up and down again. Every time the flake reaches the top it precisely kicks the spot we have practised.      When it has dissolved, we just take another one.      At least three flakes should be used up in this way, or you do the exercise as often, as we have fixed.

FLAKEQUETSCHEN (Anleitung)030

Die Zunge soll lernen, hinten hoch zu kommen.
Das muss sie können, wenn man richtig schlucken will.
Dazu tut man zuerst einmal so, als ob man heißen Tee schlürft und schluckt. Beim ‘Schluck’ soll man genau erfühlen, wo die Zunge ganz hinten mit ihrem Rücken gegen den Gaumen drückt.
Dies ist genau die Stelle, die wir bei der Sitzung eingeübt haben. Dort hinten wird nun ein Cornflake auf den Zungenrücken gelegt. Die Zunge hebt nun das Cornflake nach oben und quetscht es gegen die Stelle am Gaumen.
Da wird es dann so lange gehalten, bis es zu Flakebrei geworden ist, den man später, zum Beispiel nach der ,1-2-3’-ÜBUNG, hinunterschlucken kann. Ansonsten wird die Übung wiederholt wie verabredet.

FLAKE CRUSHER  (Pat.-Instruction)

The tongue will learn to get its rear up.      It needs that, when we want to swallow correctly.        You start with as if you want to slurp and swallow hot tea. At “Swallow” you should precisely feel, where the tongue, far back, presses its rear against the palate.        This is exactly the spot we trained at our session. Back there now a flake is positioned on the back of the tongue. Now the tongue lifts up the flake and pushes it against the POINT* on the palate.

It is held there till it has become flake mash which you may swallow, for example with the ,1-2-3’-EXERCISE. Else you do it as often, as we agreed.

 

BALLON (Anleitung)031

Die Übung dient dazu, verspannte Wangenmuskeln auszustrecken, die Gaumenmuskeln zu kräftigen, aber auch zur Übung des Lippenschlusses oder dem Glätten der Wangenschleimhaut, damit die Backenzähne sich nicht auf eine Hautfalte stellen können.

Als Ausgangshaltung legt man die Lippen leicht aufeinander wie beim ,M’-Sagen. Und nun wird der Mund zum Luftballon: Man bläst die Backen auf, als wolle man gleich ganz kräftig pusten. So viel Luft wie möglich wird in den Ballon geblasen. Halt! Die Zungenspitze dabei am * lassen. Jetzt wird langsam bis 20 gezählt. Und, falls wir das so verabredet haben, kommt nun der Trick, daß ruhig weitergeatmet werden soll. Dazu muß man hinten im Hals „zumachen“! Wenn man das beherrscht, zählt man später bis 30, 40 oder 50. Dabei melden sich die Muskeln, die noch zu schwach sind und diese Übung brauchen.                               Dreimal (oder so oft, wie besprochen) wird das ganze wiederholt.                                       Für bestimmte Zwecke kann man den Ballon auch ganz vorn hinter den Lippen oder nur an einer Seite in der Wange aufblasen. Übt man das vor dem Einschlafen, können viele Fehler während der Nacht vermieden werden.

BALLOON (Pat.-Instruction)

This exercise is good for stretching taut cheek muscles, to strengthen palate muscles but also to train lip closure and smooth out the cheek mucosa, as the molars should not squeeze on a mucosa fold.

In start position you put your lips together ever so lightly as with mouthing an “M”. Now the mouth is blown up like a balloon: You blow up the cheeks as if you are going to do a powerful puff. As much air as possible is blown into this balloon. And, stop! Leave the tongue tip on the SPOT .  Count to twenty, slowly. And, if we have agreed to that, now we do the trick with breezing on slowly. To do this we have to close the oral cavity way back in the throat. Having mastered this you later on count to 30, 40 or even 50. From this those muscles which are still too weak an need this exercise , will make their presence felt.        Three times or, as often as agreed, the whole is repeated.        For special purpose we might blow up a balloon as well quite in front, behind the lips, or only on one side of the cheeks. Training before getting to sleep may help avoiding many mistakes during night time.

 

NASENBLASÜBUNG(Anleitung)

Wie kann ich den Druck im Rachen aufbauen, ohne das Luft durch den offenen Mund entweicht? Nase zuhalten und jetzt die Luft aus der Lunge gegen den Verschluss drücken, kräftig, und bis zehn zählen. Atmen und wiederholen

Wenn diese Übung gut läuft, kommt der nächste Schwierigkeitsgrad und das ist die folgende Übung.

Man kann diese zunächst einmal solo üben. Der Trick ist aber der, beide Übungen abwechselnd zu trainieren,

NOSE BLOW EXERCISE  (Pat.-Instruction)

How can I build up pressure in the pharynx, the cave behind the tongue, without letting the air seep out through my open mouth?

Hold your nose and then press the air from the lungs against the block, with power, and count to ten. Then breath and repeat.

If this exercise is running well, it will be fore the next higher degree of difficulty and this is the following combination of exercises:

,K-K-K’-ÜBUNG (Anleitung)112

Jetzt wird der rückwärtige, weiche Teil vom Gaumen, das Gaumensegel, hinten, wo man den Weg zur Nase auf- und zumachen kann trainiert.                   Dazu atmet man kurz durch die Nase ein und stößt gleich darauf mit einem ‘K’ die ganze Luft durch den Mund wieder aus.  Anschließend wieder durch die Nase kurz einatmen und - ‘K’ - durch den Mund aus. Dabei soll man deutlich fühlen können, wie hinten im Hals das hintere Ende der Zunge oben an den Gaumen stupst. --- Einatmen (durch die Nase), ausatmen (durch den Mund), einatmen (Nase), ausatmen (Mund). Dazu zählt man eins und zwei und drei und vier. Im ganzen 15mal oder so oft, wie besprochen. Im Anschluß daran kann man sich mit einer anderen Übung (oder einfach nur so) ausruhen, bis es wieder von vorne losgeht.

Now the rear, soft part of the palate, the velum is going to be trained, there, where you can open and close the passage to the nose.

To doe this you breath in through the nose and – ‘K’ – out through the mouth. Watch the feeling, when back in the throat, the rear end of the tongue taps on the palate. – Breathe: in trough he nose, out trough the mouth, in, nose, out, mouth. Meanwhile counting: one – and – two – and – three – and – till 15, or as often; as agreed. Following to that you might rest with another exercise ( or just so), till you start again.

Die Zunge soll fühlen lernen, was ihr hinteres Ende tut.                   Wenn man ‘Kick’sagt, macht sie einen langen Katzbuckel von der Spitze am * bis hinten in den Rachen. Um das kontrollieren zu können, bekommt die Zunge ein Cornflake huckepack auf ihr hinteres Ende gelegt, da, wo wir es eingeübt haben. Anfangs muß man das mit dem Handspiegel genau überprüfen.                   Und wenn man nun ‘Kick’ sagt, fühlt man ganz deutlich die Berührung des Flakes am Gaumen.                           Also, Flake auflegen, ‘Kick’ sagen und fühlen. Zehnmal.

Wiederholt wird so oft wie besprochen.

 

' K I C K ' (Pat.-Instruction) 046

The tongue will learn to feel what its rear end is doing.        Saying “Kick” it makes a long arch of its back like a cat, from its tip at the POINT* right to the rear far back in the throat. To control this, the tongue gets a flake piggyback on its rear en, there, where we have trained it. Initially you should check the position with a looking glass. And now, when you say ‘Kick’ you will quite clearly feel the touch of the flake at the palate.

Put on the falke, say ‘Kick’, feel; ten times. Repeat as often, as agreed.

SCHNORCHELN (Anleitung)135

Der Hals soll ein Gefühl dafür bekommen, wo die Luft entlangströmt.     Das kann man am besten lernen, indem man zuerst einmal nachmacht, wie jemand schnarcht - oder wie ein Wildschwein grunzt.      Zunächst aber wird Luftholen geübt! Das geht sonst auch ohne Üben? Schon, aber nicht mit Gefühl.       Also: Mund und Nase sind offen zum Ein- und Ausatmen. Durch beide Öffnungen gleichzeitig strömt die Luft - ein und aus, ein und aus. Fühlen, wie die Luft strömt! Jetzt hält die Hand kurz die Nase zu: die Luft geht durch den Mund. Dann den Mund zuhalten: jetzt geht die Luft durch die Nase.       Das gleiche kann hinten im Rachen das Gaumensegel - jedenfalls soll es das lernen.      Darum jetzt beim Einatmen - „Schnorrrrrch“ - das Gaumensegel flattern lassen. Und ohne Schnorcheln ausatmen. Mehrmals hintereinander.

Nun als zweiten Schritt - „Schnorrrrrch“ - ausatmen und –  ohne Schnorcheln einatmen. Mehrmals hintereinander.

3.:„Schnorrrrrch“ – einatmen, „Schnorrrrrch“ – ausatmen.  Immer langsam und gleichmäßig und dabei fühlen, wie das Gaumensegel flattert. So oft, wie abgesprochen. Und den Schnorch -Stop: Zunge breit! und an den PUNKT*!

SNORKEL (Pat.-Instruction)135

The throat shall get the correct feeling, where the air is streaming past.        You may experience this quite well, when you first imitate a snoring or the grunt of a boar.        But, first of all, we will exercise breathing in. This runs by itself? Well, but not with feeling it.      

Therefore: Mouth and nose are open for letting the air flow in and out. Through both openings it streams, in and out, in and out. Feel, how it streams! Now the fingers squeeze the nose shut for a moment: The air flows through the mouth. Now cover the mouth: The air goes through the nose.        The same can be managed back in the throat with the velum – indeed, it shall be exercised. Therefore,         now, breathing  in – S n o r k – let the velum flutter. And breathe out without snorking. Several  times.        Next, as the second step – S n o r k – breathing o u t. And breathe in without snorking. Several  times.        3.: – S n o r k – breathing i n – S n o r k – breathing o u t, always slowly and regularly and at the same time feeling, how the velum flutters. A often, as agreed.

And the Snork-Stop: The tongue goes flat and to the POINT*.

 

Der PUNKT* ist der Punkt, wo die Zunge ihren Ruhepunkt hat, direkt vorn am Gaumen, 2 Millimeter der Mitte der mittleren oberen Schneidezähne, da, wo der kleine Zahnfleischknubbel ist, wo die Zunge ‚L’ sagt.   The POINT* is the point, where the tongue has its resting position, right in front at the palate, 2mm behind the middle of the upper front teeth, there, where there is the small gum bump and where the tongue say “L”.
